Comment (by bhuffman at graze.net):

 The "add chapter menu" seems to cause problems on certain DVDs.  In
 Madagasgar for example, if you hit "root menu" now it exits completely.
 Whereas "chapter menu" takes you to what I would consider a "root menu"
