 (In [13344]) Carry over [13343] from trunk.

 Fix some discrepancies in how PlaybackBox's SetPreviewGenerator(),
 IsGeneratingPreview(),  and IncPreviewGeneratorAttempts() methods
 determine
 the filename.  Previously some could end up with a preceding '/' while
 others
 didn't.

 Patch by Mark Spieth.
