Log:

Some bugs are only a very blocky American Idol finale away from being fixed.

The good intentions of IVTV_IOC_S_CODEC were translated in the switch to
VIDIOC_S_EXT_CTRLS. Now V4L2_CID_MPEG_VIDEO_BITRATE is bounded
by V4L2_CID_MPEG_VIDEO_BITRATE_PEAK as well as its natural bound.

The result of this questionable change is that the peak must be set before
the average or a new average greater than the old peak will ERANGE.
