Comment:

 (In [20997]) VideoOutputD3D improvements. The major funtionality changes
 (from Jeff Lu) are the use of hardware colorspace conversion (when
 available) and re-initialisation of the d3d device when it is lost.
 Otherwise sundry formatting changes. Closes #6618.
