 If I select the MythMusic Synaesthesia visualisation then all I see is a
 blank screen.  This occurs on both Windows and Linux and with or without
 SDL.

 Looking at the code it appears that the palette is not being correctly
 initialised.  The attached patch fixes this and also removes redundant
 code dependent on the visualisation mode, which is fixed at compile time.
