[mythtv] G400 and TV Out (again)

Unless you are finding information that I'm not...

I didn't find anything that explicitly said that you could switch head 1 and
2 but I did find the info that said you could get the
scaler/no-macrovision/accell support by use the fb method.  It was unclear
whether the tvout could also be made independent of the other head.

When I get time to do a kernel compile and install, I'll give this a try.
