[mythtv] PATCH: mythmusic setup

This patch is a speed/feature enhancement for mythmusic's setup process.

- Music files/database scanning slightly sped up (you may not notice)
- User feedback during file scanning improved
- Files are automatically scanned if you have an empty musicmetadata
table (i.e., first run); after that, choosing the "Setup" option of the
menu will trigger a re-scan.

Works for me; YMMV.
