I misread the Sync code I guess.  In any case this isn't an option for
the streaming output, so my code tries to keep the A/V sync within about
0.1 sec (~ 3 frames).  i don't think I can do anything more, since there
is variation in the audio timespan covered between two video frames.  We
also don't have the benefit of using the effective audio-rate to
compensate, since the audio and video rates need to be fixed for the
entire stream.

Anyhow, this patch has the potential to make the streaming output
unwatchable if I did something wrong.  Let's wait, and hopefully Chris or
someone will test it first.  If I don't hear anything in a couple days,
and I don't see any issues in my own personal use, then we can stick it
in CVS.
