[mythtv] DVB Code Update

Changes:
    - Nearly all of the changes Ben suggested (Thanks).
    - First attempt at a simple seektable (Thanks Isaac).

        Isaac: If you ignore the bytes_written var, wouldn't
                that work (ProcessData), or am i missing something?
                (seems the stuff in avcodec only find the position
                 of the current frame, and stores that..)

    - Are the changes to tv_rec* acceptable?
    - Some other minor things (USING_CAM wasn't there:)
