[mythtv] [PATCH] Transcoder w/cutlist updates

Here is more transcoder patch goodness.

This one finishes adding the 'x' command during recording-playback.
Currently the transcoding will always end up being done by the master
backend. I suppose I could make it so that the prefered server for
transcoding could be added to the settings, but I'm not really sure how
important that is. The code to actually do so is trivial, though I'm not
sure how to make configuration straight-forward.

The patch also prevents a program from getting queued more than once.

Also a minor fix to mythtranscode which was putting an incorrect entry in
the transcoding table.

I still haven't enabled the polling thread, but I think it is about
ready. Perhaps after any fallout from the alsa changes has happened
(haven't seen any so far) would be a good time.

Stuff on my plate to do still:
* Don't allow a manual-transcode if the cutlist is empty.
* Process cutlist without reencoding the entire file.
* Make a more direct interface for mencoder/whatever.
