[mythtv] [patch] mythscrolldialog and updated uilistbtntype

attached patch adds/enhances features:

1) new dialog type - mythscrolldialog -  with static background, scrolling
   contents and scroll arrows
2) uilistbtntype
 a) separated into new .h/.cpp files
 b) added pixmap support
 c) triple state checkboxes
 d) uilistbtntypeitem becomes a separate class - useful for subclassing
 e) misc small changes

patch is against current cvs and icons are included which go into the
default directory in the theme directory
