[mythtv] [patch] PlaybackBox

Fixes preview video playing when it shouldn't, sorry about that.
Introduced a state machine in killPlayer(), reduces average time spent in 
killPlayer() from ~400ms on my machine to ~5ms.  Preview video now waits 
500ms to start in updateVideo() instead of relying on delaying the timer.
