I've never found a way w/ cvs diff.  I keep a second copy of the CVS tree, 
make all my changes in mythtv-localdev, and use

diff -Naur -x CVS -x '.*' mythtv mythtv-localdev

to make a patch.  But maybe somebody else knows a better way?
