[mythtv] Two entries for every recording and black screen not fixed yet

Hi Daniel,

Regarding:
Ticket #742 - DVB dummy stream also recorded
you said, it's a feature not a bug.

This feature unfortunately means for me (and others too) that there're
two entries for every recorded show in the recording list. It's always
possible to start watching the file by clicking on both entries, so
both point to the same file.

I've created the BE log with -v all (like cpinkham told me to) from
when I'm watching LiveTV and from the recordings screen, probably
you're able to see what's going on.

I still see the black screen problem when I try to switch channels so
I hope the logs can help to debug the problem too (so I'll also add a
short -v playback log where it failed again to switch the channel).
