I attached another patch that protects parts of cddecoder.h with #ifdefs
that don't exist on Windows and also removes parts of cddecoder.cpp that use
stuff declared for cddecoder-darwin.

Current status of MinGW port:

mythtv compiles from current SVN (15188) with no changes,

mythplugin needs mythgoom.patch (which accidentally includes
mythplugins-configure.patch, also required) and mythmusic.patch from ticket
#4270.

Has anyone else been able to follow instructions on
http://www.mythtv.org/wiki/index.php/Windows_Port and build MythTV on
Windows?
