I looked at this ticket last night while I was debugging since John had
uploaded some backtraces.  My experience was a little different, but I
think they may be related or different symptoms of the same problem.
I didn't want to close a bunch of tickets at 4:30 in the morning though. :)
There's another where the user had a log showing what looked like the
master connecting to itself, but I can't remembe the ticket of the top
of my head.

The Slow Deletes issues are probably this same thing I ran into.  The
normal immediate delete code doesn't update the inuseprograms table, so
it wasn't affected by the issue.
