MythTV isn't doing anything special.

_Some_ TV Tuner cards have the capability to use the "btaudio"
module.  This will allow you to pull the audio data straight from the
tuner card.

If you can't use btaudio, then you must use the patch cable approach.

Unless you have an audio card that has multiple ADC's on it, you're
going to need one for each patch cord.  You will _not_ be able to use
Line-In connected to one TV Tuner, and something like Mic-In for
another one and expect that you'll be able to use both
simultaneously.
