Your experience regarding where the image shows up
versus what's plugged in
is the same as my experience with my GForce2-MX400 -
TV out defaults to ON
only if it's the only thing present.

But you should have an OK image during boot (OK
includes aspect ratio not
quite right, ragged left and right borders).  Is the
TV PAL and your card
defaults to NTSC or vice-versa?  But once X starts,
the system will try to
use your X config settings and this may screw up the
TV output.

Once you get a stable image to start, two things
helped me: editing
/etc/X11/XF86Config-4 per the nvidia driver README
(see the special sections
on multiple heads and TV out) and fiddling with nvtv. 
I have twinview
enabled in XF86Config-4, so once X starts there's an
image on both screens.
>From there, I have to do multiple steps with nvtv that
I've yet to simplify
or automate, but ultimately a very nice image is
possible.  With a given TV,
you may not get ideal overscan with any nvtv setting,
but you should be able
to get overscan to be more than necessary and then
shrink and re-position
with Myth's GUI size/position settings.
