The Hush is a good selection for silent operation. 
But it is very expensive, something like $1000 for a
moderately loaded system.  

I use a 1Ghz Nehemiah system as frontend/backend. 
Harddrives are not a problem if you get a Samung
(cheapest 5400rpm) or Seagate barracuda VI (7200rpm). 
The only thing you have to deal with is the cpu fan,
which is louder than I liked.  I modified a stock
Morex case 2699 and put in a Shuttle PH4 Heat pipe
heatsink.  I had to cut down the heatsink for it to
fit on the minitx mobo.  Not for the faint of heart. 
I used bolt cutters...I would suggest a dremel for a
cleaner job. 

The system is silent.  I use a G200, so I assume a PVR
250 would work just as well. LiveTV works fine.
