Hm... I had that same problem at one point. I can't recall what exactly
it was that fixed it, but it was not a problem anymore with this setup:

Section "Device"
	Identifier "Videocard0"
	Driver "nvidia"
	VendorName "Chaintech"
BoardName "nVidia GeForce 4 MX 440"
	Option "RenderAccel" "1"
	# TV Out Setup
	Option "TwinView"
	Option "TwinViewOrientation" "Clone"
	Option "SecondMonitorHorizSync" "30-50"
	Option "SecondMonitorVertRefresh" "60"
	Option      "MetaModes" "1024x768, 1024x768; 800x600, 800x600;
640x480, 640x480;"
	Option "TVStandard" "NTSC-M"
	Option "TVOutFormat" "SVIDEO"
	Option "TVOverScan" "0.6"
EndSection

I want to say that it was the removal of the "ConnectedMonitor" setting
that fixed it. Instead, I let the card auto-detect the displays, and
the problem went away. Can't test it anymore though (at least not very
easily), because I'm now just using VGA -> Component video (though
under this setup, I can't ever use VTs; even the bios screen is
scrambled, so nada until X loads, but the picture quality is worth it).
