Have you checked out Antec's Overture

They also say on their website that they have a super quiet powersupply fan.

Subject: [mythtv-users] good, full ATX, HTPC cases
I thank all who had the patience to answer my questions till now. :-)
...so it seems that all my problems could be solved by an HTPC case
accepting full-size ATX mobos... I didn't know there were any
on the market. As to the suggestions received here, and a little goggling
I did, I've found at least three cool full ATX HTPC cases. Here are
the links, in case somebody else is interested:
- Coolermaster ATC-613B (estinguished?)
http://www.digitalconnection.com/Products/Cases/atc613.asp
- Silverstone SST-LC03B
http://www.silverstonetek.com/products-lc03.htm
- Kanam HT400B
http://www.kanam.co.kr/
BTW, does anybody have further suggestions for HTPC cases
in this category? :-)
Franco
