[mythtv-users] QMYSQLResult::data: column 3 out of range

With no solutions to why I can only see one channel I decided to drop
the database and see if I could get more channels with a fresh database.
So I dropped mythconverg and recreated it using the script:
mysql -u root -p < /usr/share/doc/mythtv-0.16/database/mc.sql

I ran mythtvsetup and setup the tuner cards and video source. 
Then I ran mythfilldatabase. 

When I start the backend I get this:
Running as a slave backend.

Don't remember it ever saying "slave backend"...? (is this normal?)

If I check the channels in channel editor there is no channel 3, it goes
from 2 to 4. If I  look at a channel in the editor and then escape out
of it, I get this error scrolled across the terminal window running
mythfrontend:
QMYSQLResult::data: column 3 out of range

If I try to go to mythweb I get this:

Unexpected response to MYTH_PROTO_VERSION 'MYTH_PROTO_VERSION 13':

I'd really like to get my box back up and running and the fustration
level is getting high. Any help is appreciated.
