Well, for once: the pincushion problem cannot be caused by MythTV or X.
This must be a problem with the TV set. You probably just don't notice 
it during normal TV view as that is overscanned all the time.

Most people run TV-Out with a 800x600 resolution as it comes closest to 
the standard TV signal and most TV-Out encoder chips do a rescale on the 
output anyway (some do interlace and are able to handle the full 576 or 
625 lines of the NTSC/PAL)

Your refresh rate doesn't matter for the TV-Out signals as it will 
always be 50 or 60 Hz due to TV standards (PAL/NTSC) it will only affect 
the connected monitor on the standard VGA output.

For tweaking the picture you can try xvidtune too, it comes standard 
with most X installations. Just set it to auto and try 
widening/narrowing the picture or shifting it left/right. Once you found 
some proper settings you can display the suggested Modeline parameters 
and create a custom modeline for use in the XF86Config-4 file.
