Hauppauge PVR-350
AMD Athlon 1GHz
~750mb RAM
Many gigs of space
nVidia NV5 (Riva TNT2/TNT2 Pro) AGP card
MySQL v4.1.13 (Slackware-packaged)
MythTV 0.18.1

I've progressed through HowTos and READMEs, and am running mythtv-setup.
I get through the General section just fine.  Capture Cards seems OK:
Select the PVR-350 type and it auto-sets Video Device (correctly,
/dev/v4l/video0).  Changed default input from Composite 0 to Tuner 0,
click Finish.  Now the screen shows "[ 0 : 00:00:00 ]".  So I hit "E"
to verify the settings, and get this:
   Card type: 0
   Video device: /dev/v4l/video0
   VBI device: /dev/vbi0
   Audio device: /dev/dsp
   Audio sampling rate limit: (None)    [ ] Do not adjust volume
   Default intput: Composite 0
I can change Type, but it won't save it.

If I go on to Video Sources, I can set up Zap2It but it doesn't "stick".
   Name: Antenna
   XMLTV grabber: NA (DataDirect)
   User ID: xxxxxxx Password: yyyyyyyy
Leave the rest default, hit Finish.  The list shows "0".  Hit "E".  It
tries to fetch the lineup, then shows:
   Name: 0
   User ID: xxxxxx  Password: 0
   Data Direct Lineup: 0
   Channel frequency table: 0

Input connections shows *nothing*.  Channel editor is useless to me, so
I don't know if it is behaving or not.  When I exit the program, I see
   QTime::fromString: Parameter out of range
   Couldn't open 00:00:00 to probe its inputs.

I've got the IVTV modules loaded, and the address of the card is 00:0c.0
(00:12:00?).

Once I've exited the setup, I can't get back in.  Re-running
mythtv-setup only gets the first two questions ("Clear capture card
settings" and "Clear program data/channel settings").  No matter what I
answer, it pauses for ~6 seconds before going to the main menu, where
all control is gone.  The only thing I can do is ALT+TAB back to the
Konsole window and CTRL+C it.  The window is filled with the same QTime
error as before, and one "Joystick disabled" message.

I have to tell MySQL to drop the mythconverg database before setup will
"work" again.

If I had to guess, I'd say Myth is trying to pull data from other tables
to fill in the fields that show "0", but getting the syntax wrong.  Any
and all help would be appreciated!
