I made all my changes through the GUI so have never actually looked in
the config file. I did have to set that option to Pass Through in the
gui panel though so it makes sense.

Don't know. I don't watch DVDs that often so haven't really noticed
the volume diff. Most of my mythtv usage is watching live/recorded tv
or mythmusic which all appear to have similar output volume levels. I
never use any of the myth/xine volume controls anyway.

Do you still need any config files?
