[mythtv-users] Mythtv setup total failure - how to start over?

I think the issue is how to uninstall all the myth code and reinstall thus 
making the machine myth free
I never did find any discussion on where all the code is put during the 
install phase. A uninstall script would be useful at this point.

You aren't giving us much to go on.  I would yank out the other
capture card and put the PVR in, in order to isolate the
configuration.  Please repost with more info like what ivtv version
you are trying use, kernel version, etc. the output of dmesg and
/etc/modprobe.conf would also be useful.
