A couple things to try:

1). Run mythbackend from a commandline with debugging output on, so you can
see the output of the directv.pl script
2). Make sure when you test the script it's with the same user the backend
runs with.
3). Related to #2, check permissions on the serial device (/dev/ttyS0 or
whatever) to make sure that isn't the problem.
4). Check the path to make sure the right copy of the script is being run,
and that it is being found. You may even want to try putting the full path
to the script in the myth setup.

Hopefully one of those will get you closer to a solution!
