Would running the file through mencoder interfere in any way with mythtv
using the file, commercial skipping, etc? If I just wanted to create an
mpeg file outside of mythtv I'm sure this would be easy to do - but if I
were doing that I'd probably just encode for DVD burning.

I'm looking for some way to keep the file usable within mythtv in the
recorded programs list.

If mythtv can handle having its video files run through an external tool
like mencoder I imagine it would be a straightforward matter to write a
script to take a passed filename, transcode the video only, and then
replace the original file with the new one. This could then be set up
as a user job.
