I have the 'Default' setting on NTSC. Then in the Channel Editor in 
mythtv-setup the digital channels are set to ATSC.. This makes sense for 
me as there are far more analog channels than digital ones.

Sounds like you have the NTSC module loaded.....Do a modprobe -r cx8800 
followed by a modprobe cx88_dvb and you will now (should!) have a 
/dev/dvb/adapter0 entry.

If you really want digital, including HD (and don't confuse them) then 
you need the cx88_dvb module loaded.

Yes If you are using the coax input to the HD card.

Go to labs.zap2it.com and set up your channel lineups. You want an 
analog to feed the PVR, and a Digital.
Go into mythtv-setup.
Set up the cards.
Create your source lineups, choosing Analog for number 1 and Digital for 
2. You can select Retrieve Lineups from Source here and it will do a 
mythfilldatabase. DO NOT DO A CHANNEL SCAN.

Associate the cards with their sources.... PVR <-> Analog..... HD3000 
<-> digital.

You can check things in the Channel Editor and should set the Type to 
ATSC for the digital channels.
Then exit mythtv-setup and run mythfilldatabase again.

If you do a channel scan at any point before here, you can easily end up 
with duplicate channels in your tables...
