After metadata disabling file browse mode should be the next thing worth
trying. (Browse mode will attempt to iterate over MythMediaMonitor devices,
if you enter the video manager and that scan is fast, well, it is file
browse mode that is slow)

Assuming you have a reasonable number of entries in the DB, and well, it
shouldn't really be the problem.
