That channel number looks suspicious. Try redoing the scan,
with 'delete' being the existing channel treatment and select
"_" as the major minor separator. Also, make sure your video
source is not connected to another card (if that card input
receives a slightly different set of channels, you can get
untunable channels added to this card input's lineup).
