I got it from the Nvidida readme, but after reading it again, I think I 
just got lucky that it worked. I was going off this section:

Valid modes are given a unique name that is guaranteed to be unique 
across the whole mode pool for this display device. This mode name is 
constructed approximately like this:

     <width>x<height>_<refreshrate>

(e.g., "1600x1200_85")

I also just found a utility that is supposed to generate valide VESA 
modelines based on resolution and refresh rate. Search your system for a 
command called 'gtf'
