Apologies if I'm being dense.  I saw your activity on it two months
ago but your comment indicates that was simply to slowly instead of
immediately come out of pause, not the main patch of 3486 which is a
total change of API.

Although I'm a C/C++ programmer by trade, I'm totally unfamiliar with
the code, but if I'm correct that at
http://svn.mythtv.org/trac/attachment/ticket/3486/3486-v2.patch the
green is the added and the red is the deleted (as comments further
down in the code indicate), libs/libmythtv/videoout_ivtv.h should have
very near the top "#include <linux/fb.h>."  My admittingly aging eyes
don't see it at
http://svn.mythtv.org/trac/browser/trunk/mythtv/libs/libmythtv/videoout_ivtv.h.

If I'm wrong about any of this, feel free to mock me (-; as my
problems are elsewhere and I'll keep digging.
