Well the first thing to try is "Delete ALL capture cards" and "Delete
ALL video sources" and re-add and reconnect inputs.  Make sure you use
"Delete ALL" button instead of Deleting each separately.
Something is mis-configured and the best way to fix that is to start
from scratch.
