Just as a follow up, I tried making the analog portion of my pcHD5500 
card an MPEG2 capture card, an MJPEG capture card, and then put it back 
onto the default "Analog V4L Capture Card" (all from within 
mythtv-setup) and got the same results.  The MJPEG and V4L setups give 
me green static and no usable picture in LiveTV, and the MPEG2 setup 
goes into LiveTV, sits there for about 5 seconds, then returns back to 
the main menu.

I can't be the only one having an issue.  Is there any other setting I 
might be missing?  I guess I'm a little confused as to why you asked why 
I'm using V4L when that's really the only option that MythTV gives me 
for this card.
