I have this...

        AuthType           Basic
        AuthName           "MythTV"
        AuthUserFile       /var/www/htpasswd
        Require            valid-user
        BrowserMatch       "MSIE"      AuthDigestEnableQueryStringHack=On

...then just create the /var/www/htpasswd file using htpasswd.

Assuming you are running myth 0.21 you should also read the README for
mythweb as it advises against using .htaccess files now.

Q: Where did .htaccess go?
A: .htaccess has been replaced by apache- and lighttpd-specific configuration
   files.  Your old .htaccess file should still work fine, but I encourage you
   to use the new conf file, since it reduces some of the load on the webserver,
   and takes advantage of some features not accessible through .htaccess.
