For the second question:

Main reason is that often there are no details for the episode and then myth 
tries to record all of them since they don't know they are duplicates.   There 
are some channels that never seem to provide episode details for shows, and 
there are also certain channels that don't provide the correct details (5 with 
CSI: Miami reruns at 11:30 on Sat is pretty much always wrong).  With the reruns 
they are wrong quite a lot, with the prime time certain stations seem to only 
update the day before it runs.

I would try doing a update early in the day for that day, that has improved my 
luck, I have noticed that some shows get their details provided until just 
before they are running and look like duplicates until that update gets there.

I try to avoid 5, their weather people are overly excited to be on the air for 
the most minor storms, and don't restrain themselves from preempting entire 
shows for just about any storm.
