You are experiencing almost verbatim what I was having to deal with. 
Using the dvd player did nothing more than prove that the pc is sending
an empty audio stream to the tv.  This was covered in some detail a
couple weeks ago on this mailing list, I would post a link, but the
mythtv site is down right now for some reason.

You are going to have to use the info at:  http://analogbit.com/node/23
to get rid of the extension block, then add the custom modelines to xorg
that can be grabbed using either X in verbose mode, monitor asset
manager, or powerstrip.  The custom mode lines will be used in place of
the information in the extension block so that the picture is not jacked up.
