It looks like your HD-PVR is running properly now. I have had to unplug 
power on mine sometimes to get them to reset. When i do, I unplug USB, 
then power. Then I plug the power in - wait for the orange light to go 
out, then plug in USB. Then I turn on the power. I Usually leave the 
server running the whole time.

 From a few of these errors, It looks like the user you ran mythtv-setup 
from doesn't have permission to write to /dev/video4. Either run 
mythtv-setup as root, do a chmod 777 /dev/video4 or add your mythtv user 
to the video group (gpasswd -a mythtv video). This will let you select 
the proper inputs in mythtv-setup

For the error "Channel(/dev/hdpvr): SetInputAndFormat() failed" make 
sure you don't have anything in the input field "Preset Tuner to 
Channel" This is for using a PVR-250 on a cable box from the RF output 
(ie channel 3) where you still have to use an IR blaster to change 
channels.
