Some of the parameters in those lines do not match with my mythtv database.
I have 738.0, 794.0 (no offset) and 754166670, 778166670. Also 754 is qam_16.
The best way to use scan is to have just 1 starting line and let it find the other multiplexers for you. My file is
# T freq bw fec_hi fec_lo mod transmission-mode guard-interval hierarchy
T 794000000 8MHz 2/3 NONE QAM64 8k 1/32 NONE
This way there is only 1 line to get right.

I would use the scan utility to create the channel list for you. I don't think the mythtv full scan can be used when you are in the middle of a number of transmitters especially when they have all been turned up to full power.
