--refresh-today couldn't cause this to work differently.  Most likely 
you have a broken input connections configuration that's causing strange 
behaviors.

Best approach for cleaning up everything after a major change is the 
video sources part of: 
http://www.gossamer-threads.com/lists/mythtv/users/264034#264034 .  It's 
the only way that completely prevents breaking your configuration due to 
improperly dealing with old channel info.

The only way to remove a channel from MythTV configuration is to delete 
it using the MythTV channel editor (select channel and hit D).  The 
mythfilldatabase argument--if you read the description you posted--has 
nothing to do with the MythTV channel configuration.  It's all about the 
Schedules Direct configuration.
