The XFCE / Mythbuntu 9.10 had a Pulse Audio Mangler running that would
intercept certain remote codes and respond by adjusting volume. I've
disabled it in the XFCE Startup programs and I believe others have
too.

Can you run alsamixer in a window and see the levels changing?
