I missed the start of this thread so if this has already been covered
I apologise.

Before starting mythfrontend enter the following command

export DEBUG_PULSE_AUDIO_ALSA_EMULATION=1

You will still get occasional sound dropout but not mid program only
at the start of a new playback.

The above applies to .23 fixes only and I'm using it with pulseaudio
and spdif passthrough.
