this sounds like the problem i used to get when my second tuner wasn't 
working properly. It used to trip up the system until a reboot.

Can you force the error or is it entirely random?
For me it was fluctuations in input signal that caused the card to freak 
out (signal dipping too low whilst recording). The tuner / system got 
into some kind of 'funny' state that wasn't obvious in the logs etc.

I could force it by putting attenuation on the input to the card.
In the end I got a better antenna and amp to ensure a high signal strength.
