There have been various anecdotal reports about the HDHRs not
working well with some switches (either due to IGMP issues
(resolved at your firmware level), or 802.3az ("green ethernet")
issues).  Note that some ethernet nics on servers can also
have problems with 802.3az (realtek; just say no to realtek).

When the HDHR fails, does it respond to web requests?  Is
there anything interesting in the logs?

While the Prime power supplies seem slightly better, power
supply issues are always possible (and can result in random
lockouts).

If you have a more advanced switch(*), you could set up a
port on your switch to mirror the traffic on your HDHR,
and see if there are anomalous issues occurring.

(*) Where advanced, in this case, usually means a
switch targeted to enterprises and manageable.
